Medical Case Manager- Full-Time

We are seeking a dedicated and compassionate Registered Nurse Case Manager for the North Alabama, Birmingham and Montgomery regions. In this role, you will be responsible for coordinating patient care for worker's compensation cases, ensuring that patients receive the appropriate services and support throughout their healthcare journey. You will work closely with patients, families, and interdisciplinary teams to develop and implement individualized care plans that promote optimal health outcomes.

Responsibilities

  • Conduct comprehensive diagnostic evaluations to assess patient needs and develop tailored care plans.

  • Collaborate with healthcare providers to coordinate hospital medicine and ICU services as necessary.

  • Provide education to patients and families regarding medical terminology and treatment options.

  • Advocate for patients in various settings, including pediatrics and geriatrics, ensuring their needs are met.

  • Maintain compliance with HIPAA regulations while managing sensitive patient information.

  • Monitor patient progress and adjust care plans as needed based on ongoing assessments.

  • Facilitate communication between patients, families, and healthcare teams to enhance the quality of care.

Experience

  • Current Registered Nurse (RN) license in good standing

  • 3 to 5 years of clinical experience in the areas of intensive care, orthopedics, trauma/emergency, home health and occupational health care

  • Familiarity with diagnostic evaluation processes and hospital medicine practices

  • Strong knowledge of medical terminology relevant to various age groups, including pediatrics and geriatrics

  • Excellent communication skills, both verbal and written, with the ability to engage effectively with diverse populations

  • Advanced computer skills

  • This is a work-from-home opportunity; computer, phone and other necessary equipment will be provided

  • Certification- CCM, COHN, CRRN, TNCC, CDMS, is desired

  • Prior work in workers compensation experience preferred

24/7 Occupational Triage Nurse- Work from Home

The D&D Triage Nurse is the first contact for a worker injured on the job.  You would be working from home using our call center phone system.

  • On call in shifts

  • Uses a provided, secure computer to run established protocols and algorithms with D&Ds Triage Navigator software to gather information from the caller

  • Assess and triage to the appropriate level of care

  • Communicate with the provider

  • Create and send reports to the client/employer

  • Must have internet connection

  • Strong knowledge of medical terminology relevant to various age groups, including pediatrics and geriatrics

  • Excellent communication skills, both verbal and written, with the ability to engage effectively with diverse populations

  • Advanced computer skills

  • This is a work-from-home opportunity; computer, phone and other necessary equipment will be provided

Telephonic Case Management

This position combines the duties of the Case Manager with the telephonic work-from-home aspects of the triage nurse.

  • Primary duties are to communicate with the injured worker, the providers, employers and adjusters/clients in a time critical manner

  • Coordinate appointments, obtain treatment plan, work restriction information, etc., as needed

  • Create reports for adjuster/client

  • Must have internet connection

  • Strong knowledge of medical terminology relevant to various age groups, including pediatrics and geriatrics

  • Excellent communication skills, both verbal and written, with the ability to engage effectively with diverse populations

  • Advanced computer skills

  • This is a work-from-home opportunity; computer, phone and other necessary equipment will be provided
